Information you provide
The enquiry form asks for your name, email address, mobile number, service or waitlist choice, preferred contact method and time, an over-18 confirmation, and your contact and marketing choices. You may add a short note. We use this information to answer your enquiry, arrange an appointment or contact you about the waitlist or product launch you selected. If required contact information is missing, we may be unable to respond.
Please keep clinical information separate
Your service choice or note may reveal information about your health. Please use the note only for practical arrangements and do not include symptoms, diagnoses, medication or medical history. Clinical information is handled separately as part of the clinical process. This form is not for urgent or emergency help.
Why we use your information
For appointment enquiries, we use the information necessary to take steps you request before entering into a service agreement. For other enquiries and website security, our legitimate interests are responding to you and operating a reliable, secure website. We use consent for optional marketing emails. Choosing not to receive marketing does not affect your enquiry or care.

The website is hosted on Webstudio Cloud. Submitted enquiries are delivered by email to the clinic for handling. Hosting, form-delivery and email providers process information needed to provide those services. Website requests also involve technical information such as IP address and browser/request details. We do not sell your enquiry information.
International processing
Webstudio, Inc., a US company, provides our website hosting and form-delivery service. The website also uses Cloudflare for delivery and security. These services may process personal information outside the UK, including in the United States.
How long information is kept
We keep enquiry information for the period needed to handle the enquiry and any resulting appointment, complaint, legal obligation or legal claim. These factors determine how long information is needed; clinical records are handled separately. We keep marketing preferences as needed to honour your choices, including a request not to receive further marketing.
Your choices and rights
You can ask for access to your personal information or correction of inaccurate information. Depending on the circumstances, you can also request erasure, restriction or a portable copy, or object to processing.
